It seemed like Pageants allows girls who lost their rival pageants win but wont allow girls who won before another win.

Well, technically it was achieved by Helen Morgan of Wales. As the 1st runner up in Miss Universe 1974, she was the only eligible girl to take over in case Amparo Muñoz of Spain could not fulfill her duty (which actually happened). She gave up her crown. But the only girl who could inherit the title, Helen Morgan, was also dethroned as Miss World 1974 few months before Amparo gave up her crown. Hence, they could not give it to Helen.
So, technically, Helen Morgan achieved this distinction but it has not become official due to to those events, ironically are also called ’technicalities’.
